With Assertive Community Treatment (ACT), personalized support is always within reach. An interdisciplinary team offers:
- Psychiatry, therapy and psychiatric nursing
- Treatment planning and goal development
- Vocational support
- Co-occurring disorder interventions
- Peer support
- Psychoeducation for individuals served and their families
- 24/7 crisis assistance
This community-based program uses a holistic and “all-inclusive” model to provide a wide range of services through an interdisciplinary team. Services are highly individualized and accessible, often provided in homes or community settings 2-3 times per week. The goal of ACT is to promote stability, reduce barriers to care, and enable individuals to engage with less restrictive, community-based supports while addressing even the most complex mental health needs. Our intention is to support individuals in achieving independence, integrating into the community, and advancing their long-term recovery journey.
Adults age 18 and older may benefit from ACT who:
- Have been diagnosed with schizophrenia, schizoaffective disorder, major depressive disorder with psychotic features, or bipolar disorder with psychosis.
- Have had multiple hospitalizations
- Have difficulty with living independently
- Have difficulty working competitively
- Have substance abuse issues
