Adult Rehabilitative Mental Health Services (ARMHS) focus on developing life skills essential for daily living. Our Care Coordination ensures that all aspects of your treatment are seamlessly integrated for optimal results.

Adult Targeted Case Management (ATCM)

Our Adult Targeted Case Management (ATCM) program provides individualized, compassionate support for adults with Severe and Persistent Mental Illness (SPMI) facing mental health challenges who need help navigating essential services. Through this program, our experienced case managers work closely with clients to foster long-term well-being and independence by identifying their specific needs, connecting them to valuable community resources, and supporting them in reaching their personal and wellness goals.

To gain access to ATCM, each client will complete a Comprehensive Evaluation and if appropriate, be recommended to case management services. Eligibility is then determined by meeting with our ATCM Intake Coordinator.

Eligibility Criteria: 18 years or older, diagnosis of a serious and persistent mental illness, with impaired functioning, and residents of Dodge, Steele, or Waseca County. If you meet the above criteria, you may qualify for Adult Mental Health Targeted Case Management services.

Adult Rehabilitative Mental Health Services (ARMHS) at Spero are designed to help adults develop and enhance the skills necessary for independent living and community integration. This program focuses on practical, day-to-day tasks and aims to improve the overall quality of life for individuals dealing with mental health issues. Our highly trained mental health practitioners work closely with clients to develop personalized plans that address specific needs, such as medication management, stress reduction techniques, and social skills development. Through ARMHS, clients gain the confidence and capability to manage their mental health and navigate life’s challenges more effectively.

Spero’s Care Coordination service ensures that all aspects of an individual’s mental health care are seamlessly integrated and effectively managed. Our care coordinators serve as a central point of contact, facilitating communication between clients, their families, and various healthcare providers. Care coordination includes assistance with scheduling appointments, coordinating transportation, and connecting clients with additional resources and support systems within the community. This service aims to reduce stress and streamline the process of managing mental health care.
