Project for Assistance in Transition from Homelessness (PATH)
When an individual is homeless, finding resources can be a challenge. Project for Assistance in Transition from Homelessness (PATH) services connect individuals with housing services and other resources needed to find safety and stability in everyday living. We provide services in Steele, Waseca, and Dodge counties.
PATH services help an individual to:
- Obtain the necessary paperwork to secure housing and create an individual housing plan.
- Work through barriers preventing an individual from finding and keeping housing.
- Apply for economic assistance, medical assistance, and social security as needed.
- Get connected to medical and mental health care, legal assistance, and other services.
**Access the Coordinated Entry system to secure housing services.
